Symbria Rx is currently seeking a Pharmacist to add to our growing Pharmacy Team in Woodridge.

Position SummaryThe primary purpose of this position is to provide pharmacy services safeguarding patient safety and effectiveness of therapy consistent with all applicable policies, procedures, and legal requirements.
LTC Experience Preferred Working Hours:
40 hrs/week alternating 1st and 2nd shift and every other weekend
Responsibilities:
o Possess knowledgeable understanding of current medication information, health care concepts and all regulations relative to the practices of institutional pharmacy.
o Interpret and assess new and refill order requests accurately to ensure appropriate therapy and maintain patient safety.
o Clarify inappropriate physician orders when necessary.
o Transcribe telephone orders accurately and review for appropriate therapy.
o Provide an accurate final check on all prepared medication to ensure completeness.
o Prepare and complete all appropriate documentation required on orders, prescriptions, controlled substance records, general workflow, etc.
o Monitor and recommend therapy adjustments based on patient status, lab work, and culture results.
o Monitor Total Parenteral Nutrition (TPN) lab work, recommend dosing adjustments when appropriate, communicate with nursing staff and prescribers, enter orders into the pharmacy software, and coordinate deliveries to communities.
o Provide physicians, nursing staff, and public with drug information such as drug selection, dosage, proper administration, etc.
o Counsel patients and review medications using audio/visual technology for tele-pharmacy.
o Maintain established goals for initial and final review productivity levels, which meets operational needs and ensures deliveries leave as scheduled.
o Model a supervisory role:
on a daily basis, in partnership with management, assume responsibilities including assigning, directing, and supervising pharmacy staff and workflow to achieve and maintain consistent and efficient processing and delivery of all prescription orders.
o Any and all duties as assigned.
Qualifications:
o Bachelor of Science degree in Pharmacy or Doctorate of Pharmacy (PharmD) from NABP accredited College of Pharmacy.
o Valid state registered pharmacist license.
o Institutional pharmacy experience preferred.
o General computer knowledge including Microsoft Office:
Word, Excel and Outlook.
o Exercise professional phone, verbal and written communication skills.
o Ability to maintain organization, prioritize and multi-task.
Exhibit initiative and reliability.
o Must be able to work holidays and alternate shifts for PTO/staffing coverages as 24/7/365 operational needs dictate.
